Volunteer Application

This application form is fairly extensive and may take you 15-20 minutes or so to complete.  Because the form is web-based, you'll want to complete it when you have a reliable Internet connection so that you don't risk losing your work.

IMPORTANT NOTE:  If a question has a red asterisk (*) at the end, it is "required"--i.e., it cannot be left blank.  If you don't have an answer for a required question, enter "N/A" or something similar.  Otherwise, you will not be able to submit the form.

IMPORTANT NOTE: You must finish the form in one sitting--you cannot start it today and come back to finish it tomorrow.

Here is a list of information you will need to complete the form:

Everyone will need:

• 3 professional or personal references who are not family
  • You'll need name, phone, and relationship to you
• 2 people to notify in the case of an emergency
  • You'll need name, phone, email, and relationship to you
• You may also want to think about:
  • Your volunteer experience
  • Your experience working with older adults
  • Any concerns you have about volunteering
  • Any skills or interests that you'd like to share with others

If you might transport members in your vehicle, you will also need:

• Your driver's license
• Your vehicle's proof of insurance card
